3. 3.Name of parent / guardian

C.) Phone Calvary Pentecostal Assembly and register via phone. (519) - 621-6310 Please have the following information ready.

D.) Come by Calvary Pentecostal Assembly main office and register in person. The church is located at 127 Hespler Rd., Cambridge, Ontario.
