Day Camp Schedule


	Registering is as easy as 1,2,3. Simply follow the steps below and that's all there is to it!
	1	Complete Registration Form Print off and complete a REGISTRATION and RULES FORM. Forms are also available at Cavlary Assembly. Be sure to read the rules and regulations, and both camper and parent must sign this sheet.
	2	Fax or Come to Calvary with your form. Bring your Registraion and Rules Forms to Calvary Assembly. Our office hours are 8:30 am to 5:00 pm. You can also fax in these forms to 519-621-6771.
	3	Payment Complete your registration with your payment. We accept Cash, Cheque, Post Dated Cheques, VISA, or MasterCard. Credit Card Payments can by made by including the info on your Registration Form. Cash and Cheque payments can be made by coming to Calvary Assembly at 127 Hespeler Road in Cambridge. If you are paying by post dated cheques, cheques must be dated for Friday prior to week of camp, and first week's payment is due upon registration.
	Tuck Shop: Stomp Day camp has a tuck shop that is open each day and sells snacks and drinks. If you would like your child to have some spending money to use at the tuck shop, you can PRE-PAY the amount of your choice on the REGISTRATION FORM, and include the amount in your Total Payment. This will save you time on the first day of camp.
